What Is a Waiting Period in Spanish Health Insurance?

A waiting period — sometimes referred to as a grace period — is a defined period of time at the start of a new health insurance policy during which not all of the policy benefits are available to you.

During a waiting period, you are technically insured, but certain treatments, specialist referrals, or diagnostic tests cannot be claimed. Emergency treatment is almost always covered from day one, but planned or routine care may not be.

Waiting periods are a standard feature of many Spanish private health insurance policies. They exist because insurers need to manage the risk of someone taking out a policy specifically to access treatment they already know they need. They are not a sign of a poor quality policy — in fact, some of the most comprehensive health insurance plans available in Spain include them.

What Kind of Waiting Periods Are There?

Waiting periods in Spanish health insurance vary by the type of treatment or procedure involved and from company to company. The following gives a realistic picture of what you might encounter in a typical policy that includes them — though the exact periods do vary between insurers.

It is worth noting from the outset that life-threatening illnesses are almost always exempt from any waiting period, meaning emergency care is covered from day one regardless.

For everything else, waiting, grace or qualification periods — as they are variously termed in Spanish policy documentation — Here are some examples but they vary from insurer to insurer:

  • Minor surgical procedures (lower complexity operations) — around 3 months
  • Psychology and mental health consultations — around 6 months
  • Complex diagnostic tests (such as MRI, CT scans and similar) — around 6 months
  • Childbirth and caesarean section (excluding premature birth) — around 8 months
  • Complex therapeutic treatments — around 10 months
  • Major surgery and hospitalisation (higher complexity operations) — around 10 months
  • Vasectomy and tubal ligation — around 10 months

As you can see, the more significant the procedure, the longer the typical waiting period. A policy with these kinds of restrictions is not necessarily a poor one — many well-regarded insurers operate on this basis for their standard policies. However, for NLV applicants, these restrictions matter enormously, which is why sourcing a policy without them is so important.

Why Many Consulates Object to Waiting Periods

This is where waiting periods become particularly significant for Non-Lucrative Visa applicants.

Spain’s consulates and embassies process NLV applications from around the world, and their requirements are not always identical. However, a significant number of consulates — including several of the most commonly used by UK and US applicants — specifically require that your health insurance policy contains no waiting period.

The reason is straightforward. The consulate needs to be satisfied that you will have access to full health coverage from the moment you arrive in Spain. A policy that restricts your access to treatment for the first six or twelve months does not, in their view, meet that requirement.

Submit a policy with a waiting period to the wrong consulate and they will likely refuse or return your visa application. This can mean delays, additional costs, and the need to source a replacement policy — sometimes at short notice.

Not all consulates take the same position. Some are less strict on this point, which may give you a wider choice of insurance providers. However, unless you have confirmed directly with your specific consulate that a policy with a waiting period is acceptable, it is a risk we would generally advise against taking.

Most Spanish health insurance policies — including those without a waiting period — contain exclusions for pre-existing conditions. Importantly, this applies not just to conditions you have been diagnosed with, but also to conditions you may have without yet being aware of them.

In other words, an unknown pre-existing condition is still treated as a pre-existing condition in the eyes of your insurer. This is an area that catches many policyholders by surprise, and it is worth understanding fully before you take out any policy.
